You require a Licence to legally practice Opticianry in Manitoba. You must complete these four tasks to apply for a Licence:
Task 1. Required Documents
Gather your required documents:
a) OAC Membership (optional)
Details are provided in the Application for Licence Form (See Task 2 below).
b) Proof of Insurance
You require a minimum of $1 million in Professional Errors and Omissions Liability Insurance coverage. Provide a photocopy of your insurance certificate.

1. Become a member of the Opticians Association of Canada
2. Visit an insurance agency
Average cost of insurance: $175 (2016)
c) Criminal Record Search Certificate
Complete a Criminal Record Check including a Vulnerable Sector Search Request, and provide the original Criminal Record Search Certificate to OOM.

OOM will accept an original Criminal Record Search Certificate that is up to 3 years old. You must submit an original certificate to OOM even if you submitted the certificate during your Application for PLAR. You will also be required to complete an Active Declaration, which includes questions regarding any previous charges for criminal offenses, when you apply for your license.
d) Passport Photo
Provide one original passport photo with the signature of a witness on the back of the photo to verify your likeness.

Task 3. Licence Fee
Prepare your licence fee:
You may pay by one of the following methods:
- Money order
- Credit card – VISA or MasterCard (To pay by telephone, call OOM at 204.222.8404)
- Certified cheque (Please make cheques payable to Opticians of Manitoba)
- Cash is NOT accepted
Task 4. Delivery
Mail or hand-deliver all required documents with your application form and licence fee to OOM:
