You require a Licence to legally practise Opticianry in Manitoba. You must complete these four tasks to apply for a Licence:
Task 1. Required Documents
Gather your required documents:
a) OAC Membership (optional)
Contact the Opticians Association of Canada (OAC) at www.opticians.ca or by phone at 204-982-6060 to obtain an application form.
b) Proof of Insurance
You require a minimum of $1 million in Professional Errors and Omissions Liability Insurance coverage. Provide a photocopy of your insurance coverage certificate.
You may acquire insurance through an insurance agency or by becoming a member of the Opticians Association of Canada. If you become a member of the OAC your membership includes $1 million in Professional Errors and Omissions Liability Insurance at no additional charge.
c) Criminal Record Search Certificate
Complete a Criminal Record Check and provide the original Criminal Record Search Certificate to the Opticians of Manitoba (OOM). A Vulnerable Sector Screening is required.
Use the Winnipeg Police Service online service Epic to request your search or contact your local police service.
OOM will accept an original Criminal Record Search Certificate that is up to 3 years old. You must submit an original certificate to OOM even if you submitted the certificate during your Application for PLAR.
d) Passport Photos
Provide one original passport photo with your signature on the back of each photo.

Complete the 2021 APPLICATION FOR LICENSE FORMs (PDF)
Task 3. Licence Fee
Prepare your License Fee for Eyeglasses or Contact Lenses.
You may pay by one of the following methods:
- Money order
- Credit card (To pay by telephone, call OOM at 204.222.8404)
- Certified personal or business cheque (Please make cheques payable to Opticians of Manitoba)
Task 4. Delivery
Mail all required documents with your application forms to OOM:
